Contestant Application Form

All completed applications must be accompanied by a non- refundable application fee of US$200.00, by Money Order or Cashier's Check Payable to: New Guard Production Company, 584 Castro Street #293, San Francisco, CA 94114.

All submitted applications are reviewed for consideration to be contestants. The producer of the event reserves the right to discreetly not accept any application in their sole discretion, therefore, unaccepted applications will be given full refund of their application fee.
